Welcome to Chatham Dental Associates, we are happy you have chosen us to serve your dental needs. Our objective is to provide superior dental care and help you feel and look your best.
We are dedicated to providing you with a pleasant visit and results that will be an excellent value to your long-term dental health. Our administrative staff is ready to help you with questions about scheduling, financial policy and insurance to make that part of the process as simple as possible.
Your initial appointment will consist of a comprehensive examination with immediate focus on any problem which is of prime concern to the patient and a review of the diagnostic materials necessary to develop treatment options for your particular situation. A complex medical history or treatment plan will require an evaluation and a separate appointment to provide treatment. In most cases, a cleaning may be performed on the first visit.
Please assist us by providing the following information prior to your consultation:
- X-Rays taken by a previous dentist, if applicable.
- A list of medications you are presently taking.
- If you have dental insurance, bring your insurance card.

Note: All patients under the age of 18 must be accompanied by a parent or guardian. Please alert the office if you have a medical condition that may be of concern prior to treatment or if you are currently taking any medication.
X-Rays: If your previous dentist has taken X-Rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional X-Rays are necessary the reasons they are needed will be reviewed with you and they can be taken at our facility upon your consent.
